1. You may obtain the New Client Paperwork by clicking here. You may print it
out so you may read, complete, and sign the forms, which cover your rights,
responsibilities, and basic demographic information. Please bring them with you
for your first appointment. I will briefly review key information as required at the
beginning of the session, and I will be glad to assist in completing any form that
you have questions about.
2. Please also print the Client Copy of Paperwork so that you have a copy of the
forms, including the privacy practices form.
Please feel free to contact me with any questions you may have.
